EQUIPMENT ORDER FORM – for USB readers, Digital Signature pads, Epson printer
CLIENT INFORMATION SHEET – is not only your request to move forward with setting up an account, it also gives us the necessary information needed to help in assessing your present configuration of equipment, peripheral services (checks, gift cards, etc…), and/or remaining term on existing Agreement with current processor.
VIRTUAL TERMINAL SERVICE ADD FORM – this form is required to activate and open Web access to the Virtual Terminal. This is a generic add/change form that covers numerous account items, but it is pre-checked for the TXP (Transaction Express) and $5.00 for the TXP Monthly web access fee. This is a single $5 fee that covers all pc’s in your office for just the Virtual Terminal. It is NOT a $5 per pc fee, nor does it cover any other web services at this time.
DIRECT DEPOSIT ACCOUNT change form (DDA) – print and sign this form to enable us to change the bank account that your deposits go to. We also require a copy of the new deposit account check back with the form. Your signature and the check copy is all we need to complete this for you
